Registered Nurse - Transitional Care

Location: Portland, ME

Assignment Duration: This is a 13-week contract position.

Schedule: Shift: 7:00 PM - 7:30 AM, Nights

Job Description: We are seeking a Registered Nurse (RN) for our Transitional Care unit. The RN will provide comprehensive nursing care to patients in a long-term and skilled nursing facility. The role requires excellent communication skills, the ability to work in a team environment, and a strong commitment to patient-centered care.

Responsibilities:

• Deliver patient-centered care in a respectful and compassionate manner.
• Monitor and assess patient conditions, including administering medications, treatments, and IV therapy as needed.
• Document patient care activities accurately and timely in the electronic medical record (EMR).
• Collaborate with the healthcare team to create and implement individualized care plans.
• Assist in the discharge planning process and ensure continuity of care.

Required Experience / Certifications / Licensure:

• Current RN license with at least 1 year of recent acute care experience.
• BLS certification is required.
• Experience with EMR charting is preferred.
