Position type available is Full-time, Part-time, or PRN
As a Physical Therapist, you will visit multiple patient’s homes each day and provide them with a full range of PT care. You must be able to work not only on scheduled times but on an on-call basis if needed.
Requirements:
Minimum of 1-3 years of experience in home healthcare
Current Illinois PT license
Current CPR Certification
Valid driver’s license

Conduct initial evaluation, assessment, prepare and implement treatment plan, reevaluation necessary to attain patient’s therapy goal. Ensure proper documentation is completed according to BIHC policy. Ensures all service provided is completed according to BIHC standards and expectations.
