Duties

• Assist patients with daily activities such as bathing, dressing, grooming, and mobility to promote independence and comfort
• Monitor and record vital signs including blood pressure, temperature, pulse, and respiration rates accurately using EMR (Electronic Medical Records) systems
• Support medication administration under supervision, ensuring proper documentation and adherence to HIPAA regulations for patient confidentiality
• Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments in memory care or assisted living settings
• Use safe lifting practices and equipment such as Hoyer lifts to transfer patients securely and prevent injury
• Maintain aseptic techniques during procedures like catheter care or wound management to prevent infections
• Collaborate with nursing staff on patient care plans, communicate effectively with patients and families, and document all interactions thoroughly in medical records

Requirements

• Valid CNA certification with current state licensure or registration
• Prior experience working in long-term care facilities, hospitals, or home care environments is highly preferred
• Demonstrated knowledge of medical terminology, vital signs monitoring, and basic life support (CPR) certification
• Experience working with diverse patient populations including geriatric patients, individuals with developmental disabilities, or those requiring hospice or ICU care
• Familiarity with EMR/EHR systems such as Cerner or eClinicalWorks is a plus
• Ability to perform heavy lifting safely and handle manual patient transfers using appropriate techniques and equipment
• Strong communication skills to effectively coordinate with healthcare teams and provide compassionate patient service
• Knowledge of infection control practices including aseptic technique and working within HIPAA guidelines for patient confidentiality
