Behavioral Health Consultant — Primary Care Team
Outreach Community Health Centers
Outreach Community Health Centers is seeking a Behavioral Health Consultant to join the primary care team under the Director of Behavioral Health Services. You will identify, triage, and assist in managing patients with mental health concerns within primary care, providing psychoeducation and developing behavioral change plans. Responsibilities include assessing patients referred by PCPs, coordinating with the care team, and collaborating with outside providers. #J-18808-Ljbffr Outreach Community Health Centers
