Full-Time RN Case Manager

BrightStar Care of Plymouth is a growing home care company with a mission to help veterans age in-home. We're looking to hire a Full-Time Registered Nurse (RN) Case Manager with compassion and experience to help us grow. This position will start with a very small case load within our primary service area, with a temporary focus to assist the DON with supervisory visits and initial assessments within our entire territory (up 35 miles from Plymouth).This is a traveling position with high autonomy! Ability to commit to at least 32-40 hours per week. No rotation on-call, no evenings and no weekends. We promise a comfortable case load with flexible scheduling (make your own schedule) to help with work life balance. Base Pay: $39-$41/hr401K plan with 4% company matchPaid Drive Time & Travel ReimbursementEmployee Referral ProgramWeekly PayWork laptopPrimary Service Areas: Plymouth, Maple Grove, St. Michael, Hopkins, Wayzata, Mounds, and Minnetonka.Other Services Areas: Maple Plain, Rockford, Delano and Buffalo.RN Case Manager Requirements:Must be Active on the MN Board of NursingMust be current on CPR CertificationMinimum of one year of experience as a Nurse, preferred but not requiredMUST BE ABLE TO TRAVELMust have Driver's License with insured, reliable transportationRN Case Manager Essential Duties:Provide in-home care for clients based on personalized plan of careOversee caregivers and perform supervisory visits and competency testingPerform and maintain on-going client case managementThoroughly document assessments and cares using eCharting systemProvide care coordination for individual clients, including coordination with other agencies, healthcare providers, pharmacies, medical supply companies, etcManage the client's nursing schedule; be prepared to fill shifts urgently if neededCover for two other case managers when they go on vacationOther duties assigned or delegated by DON, including computer work in the office if necessaryAdvance your career and join a compassionate and dedicated care team.
