Defined:
The Ryan White Outreach Coordinator is responsible for HIV outreach, testing, and linkage-tocare activities for populations at high risk for HIV infection in the Fredericksburg area region, as 
well as the day-to-day administration of FAHASS's 340B Drug Pricing Program. This position 
bridges community-based outreach with clinical program compliance, ensuring that newly 
diagnosed individuals, those out of care, and eligible patients are connected to — and retained 
in — Mosaic Care Center and Ryan White-funded services. The Ryan White Outreach 
Coordinator works in close collaboration with the clinical team, case managers, and community 
partners to advance health equity and the FAHASS mission.
Employment Standards:
● High school diploma or General Education Development (GED) required; Associate's or 
Bachelor's degree in Public Health, Health Education, Social Work, or a related human 
services field preferred.
● Minimum of one (1) year of experience in HIV outreach, community health work, case 
management, or a related field.
● Virginia Department of Health (VDH)-approved HIV Counselor certification required, or 
willingness and ability to obtain certification within 90 days of hire.
● Experience with 340B program administration, pharmacy operations, or healthcare 
compliance strongly preferred.
● Familiarity with Ryan White Part B program requirements, eligibility standards, and 
service categories preferred.
● Experience working with people living with HIV/AIDS, LGBTQ+ individuals, or other 
medically underserved communities preferred.
● Demonstrated ability to conduct outreach and build trusting relationships with 
individuals from diverse socioeconomic, cultural, and linguistic backgrounds.
● Bilingual (English/Spanish) a benefit.
Key Responsibilities Include the Following:
HIV Outreach & Testing
● Conduct HIV screening and testing using approved rapid testing protocols and VDH 
guidelines, including pre- and post-test counseling consistent with Virginia HIV testing 
laws.
● Plan, organize, and facilitate HIV testing events targeting populations at high risk for HIV 
infection, including MSM, transgender individuals, people who inject drugs (PWID), and 
communities of color.
● Identify, locate, and engage newly diagnosed HIV-positive individuals and those who have 
fallen out of care, facilitating prompt linkage to Mosaic Care Center and Ryan White 
services.
● Conduct active field-based outreach at community venues, shelters, harm reduction 
sites, LGBTQ+ spaces, health fairs, and other locations where at-risk individuals 
congregate.
● Attend and represent FAHASS at community events, partner meetings, coalitions, and 
health fairs to promote HIV awareness, prevention, and available services.
● Distribute HIV prevention materials, condoms, and information on PrEP, PEP, and harm 
reduction resources during outreach activities.
● Provide basic health education and information on sexual health, STI prevention, and 
linkage to additional FAHASS services.
● Coordinate STI screening and referral in conjunction with HIV testing activities, as 
applicable.
Ryan White Part B Outreach Services
● Provide Ryan White-funded outreach services consistent with Virginia's Part B service 
category definitions, including:
○ Identifying and making contact with HIV-positive individuals who are not in care 
or not receiving Ryan White services.
○ Providing information about and facilitating access to Ryan White-funded medical 
and support services.
○ Following up with individuals who miss scheduled medical appointments to 
support re-engagement in care.
○ Assisting individuals with navigation of the healthcare system, including 
explaining eligibility requirements and available service categories.
○ Coordinating with medical case managers to facilitate warm handoffs and intake 
for newly linked clients.
○ Documenting all outreach contacts, encounters, and referrals in CAREWare or the 
designated data system per Ryan White reporting requirements.
● Confirm and document Ryan White eligibility for applicable individuals, including income 
verification, HIV status documentation, and recertification as required.
● Screen individuals for eligibility for additional FAHASS services, including pharmacy 
services, medical case management, transportation, SNSN, and mental health services.
● Maintain accurate, timely outreach records in CAREWare, supporting monthly and 
quarterly grant reporting.
● Contribute data and narrative content to grant reports, performance measure tracking, 
and programmatic workplan goals.
● Participate in HIV care continuum analysis and quality improvement efforts, including 
identifying gaps in testing, linkage, and retention within the FAHASS service area.
340B Drug Pricing Program Administration
● Oversee day-to-day operations of the FAHASS 340B Drug Pricing Program, ensuring 
continuous compliance with HRSA program requirements, federal regulations, and 
FAHASS policies and procedures.
● Identify and recruit eligible patients into the 340B program, including:
○ Reviewing patient panels and encounter records to identify individuals who meet 
the HRSA 340B patient definition.
○ Conducting outreach to eligible patients to explain program benefits, cost savings, 
and available pharmacy options.
○ Enrolling patients and assisting with pharmacy selection, prescription transfers, 
and initial fills.
○ Coordinating with clinical and case management staff to capture eligible patients 
at the point of care.
● Support patient retention within the 340B program, including:
○ Monitoring refill patterns and identifying patients at risk of lapsing in medication 
adherence or program participation.
○ Conducting follow-up outreach to patients with missed refills, gaps in care, or 
lapsed eligibility.
○ Assisting patients in resolving barriers to access, including insurance changes, 
transportation, or pharmacy issues.
○ Tracking retention metrics and reporting trends to the Programs Manager and 
Executive Director to inform program improvement.
● Maintain program integrity by ensuring compliance with all HRSA 340B requirements, 
including:
○ Patient eligibility verification and documentation consistent with the 340B patient 
definition.
○ Prevention of duplicate discounts and diversion.
○ Monitoring of contract pharmacy activity and reconciliation of dispensing records.
○ Review of purchasing accounts and inventory records for accuracy and 
compliance.
○ Maintenance of audit-ready policies, procedures, and program documentation.
● Manage FAHASS registration, site updates, provider listings, and contract pharmacy 
listings in the HRSA Office of Pharmacy Affairs Information System (OPAIS); complete 
annual recertification on time.
● Serve as the primary organizational liaison to contract pharmacies, wholesalers, thirdparty administrators (TPAs), and drug manufacturers regarding 340B program matters.
● Conduct routine internal 340B self-audits and prepare for external audits; document 
findings and corrective actions.
● Track and regularly report 340B program revenue, savings, and utilization data to the 
Executive Director and Finance staff.
● Coordinate with clinical staff to ensure that prescribing and encounter documentation 
supports 340B patient eligibility.
● Provide ongoing staff training on 340B program requirements, eligibility standards, and 
changes to federal guidance; update internal procedures as needed.
● Stay current on HRSA 340B program guidance, proposed rule changes, and industry 
developments; proactively communicate implications to leadership.

● Demonstrated commitment to affirming, culturally responsive service delivery for 
LGBTQ+ individuals, people living with HIV, and other underserved communities.
● Ability to work both independently in the field and collaboratively within a 
multidisciplinary team.
● Excellent verbal and written communication skills, including the ability to communicate 
clearly with patients, community partners, pharmacies, and regulatory contacts.
● Proficiency with Microsoft Office; ability to learn and navigate data systems such as 
CAREWare, OPAIS, and related platforms.
● Ability to organize and prioritize multiple responsibilities — including field-based 
outreach and regulatory compliance duties — while maintaining accuracy and attention 
to detail.
● Possession of a valid state driver's license and personal vehicle insurance required; 
routine travel within the FAHASS service area is expected.
● Flexible scheduling required; evening and weekend availability will be necessary to 
support outreach events and testing activities.
● Ability to maintain confidentiality at all times and work effectively within a team 
framework.
● Perform other duties as requested by the Executive Director in support of FAHASS 
policies and mission.
