REGISTERED NURSE

Job Title Registered Nurse (ACT/ACT PLUS) Job Overview The Registered Nurse (RN) is a key member of the Assertive Community Treatment (ACT) / ACT Plus multidisciplinary team, responsible for delivering comprehensive nursing care and clinical support to individuals with severe and persistent mental illness, co‑occurring disorders, and complex healthcare needs. The RN provides community‑based nursing services that support recovery, rehabilitation, health maintenance, medication management, and independent living, while working collaboratively with behavioral health professionals, medical providers, families, and community partners. Responsibilities Clinical Assessment & Nursing Care Conduct comprehensive psychiatric, clinical, and physical health assessments. Evaluate participant health needs and develop nursing interventions to support treatment goals. Monitor participant physical and behavioral health status. Provide direct nursing care and health‑related services in community and office‑based settings. Assess participants for changes in physical, psychiatric, and behavioral conditions. Identify actual or potential health concerns and communicate findings appropriately. Maintain accurate clinical documentation and nursing records. Medication Management Administer and manage medications and prescribed treatments. Provide medication education to participants and families. Monitor medication effectiveness, side effects, and adherence. Administer injections and other ordered treatments as prescribed. Maintain medication inventories and ensure proper storage, handling, and documentation. Ensure compliance with medication administration regulations and standards. Coordinate prescription refills and medication‑related services with pharmacies and providers. Community‑Based Services & Recovery Support Provide community‑based nursing and support services to participants. Deliver in‑home services, including medication training, activities of daily living (ADL) training, home management support, and health education. Support participants in achieving recovery, wellness, and independent living goals. Promote self‑management of health conditions and treatment adherence. Assist participants in navigating healthcare systems and accessing services. Health Education & Counseling Educate participants regarding mental health conditions, physical health conditions, medications, disease prevention, wellness practices, and recovery strategies. Provide supportive counseling and encouragement. Assist participants in understanding treatment recommendations and healthcare plans. Provide consultation and support to family members and caregivers. Crisis Intervention & Care Coordination Coordinate emergency and crisis intervention services when necessary. Assess urgent clinical situations and initiate appropriate interventions. Participate in crisis prevention and response planning. Coordinate medical and behavioral healthcare services. Ensure follow‑through on medical appointments, treatments, and provider recommendations. Communicate treatment changes and significant clinical information to team members and providers. Participate in on‑call responsibilities as assigned. Interdisciplinary Team Collaboration Participate in treatment planning meetings and interdisciplinary case conferences. Collaborate with psychiatrists, therapists, case managers, peer specialists, and other providers. Contribute nursing expertise to individualized treatment plans. Support person‑centered, recovery‑oriented service delivery. Resource Coordination & Advocacy Assist participants in accessing healthcare, housing, employment, and community resources. Coordinate referrals and linkages to external healthcare and support services. Advocate on behalf of participants to improve access to services and supports. Support participant rights, self‑determination, and community integration. Compliance & Quality Assurance Ensure compliance with nursing practice standards, licensing requirements, medication regulations, agency policies, and behavioral health standards. Support infection prevention and disease control activities. Maintain relationships with medical providers, pharmacies, hospitals, and ancillary healthcare services. Participate in quality improvement initiatives and program development. Staff Training & Support Provide consultation, support, and education to interdisciplinary team members. Train and supervise unlicensed staff regarding health‑related procedures and care standards. Promote safe and effective healthcare practices throughout the program. Qualifications Graduate of an accredited School of Nursing. Current unrestricted Registered Nurse (RN) license in the State of Delaware. Successful completion of required background investigations and security clearances. Pre‑employment Tuberculin Skin Test (TB Screening). CPR Certification. Preferred Qualifications Bachelor of Science in Nursing (BSN). Minimum one (1) year of nursing experience. Experience working in behavioral health, community mental health, Assertive Community Treatment (ACT), psychiatric nursing, substance use treatment, or integrated healthcare settings. Licenses & Certifications Current Delaware Registered Nurse License. CPR Certification. Valid driver’s license with acceptable driving record. Ability to maintain security clearance requirements, including Delaware Department of Correction approval where applicable. Key Skills & Competencies Strong nursing assessment and clinical intervention skills. Knowledge of psychiatric and physical health conditions. Strong medication management and patient education skills. Excellent verbal and written communication abilities. Ability to work independently within community settings. Strong organizational and documentation skills. Effective crisis intervention and de‑escalation abilities. Ability to work collaboratively within multidisciplinary teams. Strong problem‑solving and decision‑making skills. Knowledge of recovery‑oriented and trauma‑informed care principles. Physical Demands & Working Conditions Requires sitting, standing, walking, and driving for extended periods. Providing community‑based healthcare services in participant homes and community locations. Reaching, bending, lifting, stooping, twisting, and climbing stairs. Conducting nursing assessments and interventions in varied environments. Responding to emergencies and crisis situations. Ability to occasionally lift, move, or carry up to 50 pounds. Community‑based behavioral health settings, participant homes, office and clinic settings, healthcare facilities, and multiple service locations throughout Delaware. Safety & Regulatory Knowledge Bloodborne Pathogens, Right‑To‑Know, HIPAA, Infection Prevention & Control, Medication Safety Standards, Fire and Safety Regulations, Workplace Safety Procedures, Emergency Response Protocols. Maintain and abide by OSHA requirements and guidelines. Observe all fire, safety, fiscal, office, human resources, personnel, and agency policies and procedures. Attend required trainings, meetings, certifications, and professional development activities. Support person‑centered, trauma‑informed, and recovery‑oriented care practices.
