This position will be performed remotely, allowing for a flexible work environment while enabling you to impact lives positively from your own home. As part of a hospice center, you will have the unique opportunity to work with clients experiencing life-limiting illnesses, their families, and caregivers. You will receive support from a dedicated team of healthcare professionals, including nurses, doctors, and other social workers, fostering a collaborative approach to patient care.

  • Conduct individual and group therapy sessions to provide psychological support and therapeutic interventions tailored to clients’ needs.

  • Develop and implement personalized treatment plans in collaboration with clients, tracking progress and adjusting goals as needed.

  • Advocate for clients, assisting them in navigating social services, financial aid, housing support, and additional community resources.

  • Provide crisis intervention services for clients facing acute mental health emergencies, including suicidal ideation or severe trauma responses.

  • Participate in workshops and community outreach programs focused on mental health education and awareness.

  • Maintain accurate and confidential patient records in compliance with HIPAA and state regulations, ensuring all documentation is current and thorough.

  • Engage in ongoing professional development through mentorship programs to enhance clinical skills and personal growth.

Patient Population / Client Focus\n

Your primary focus will be on individuals and families impacted by chronic illness, grief, and the mental health challenges associated with navigating end-of-life care. This population may include patients facing terminal illnesses, their caregivers, and families coping with loss. Your role will encompass providing not only psychological support but also practical guidance to help them manage their situations with dignity and resilience.

  • A Master's degree in Social Work from an accredited program.

  • Current Pennsylvania LCSW license, allowing for independent practice.

Compensation and Benefits\n
