Location and Work Details Location: LIFE Geisinger Wilkes-Barre. Shift: Days. Country: United States of America. Scheduled weekly hours: 40. Worker Type: Regular. Exemption Status: Yes. Job Summary Responsible for the development and implementation of home care services for participants, including coordination of personal care services, durable medical equipment needs, and nursing home care. Key Responsibilities Coordinate implementation of all home care services, including nursing, physical, occupational, and speech therapy, personal care services, dietary needs, and durable medical equipment, ensuring quality service for participants. Ensure appropriate authorizations are obtained and entered into required software programs. Conduct initial nursing assessments and revaluations of participants, including home assessments in collaboration with social workers. Present assessments to the interdisciplinary team at scheduled meetings and collaborate on creation and revision of care plans. Assess home care service functions, make recommendations and offer solutions to revise or modify plans of care to better meet participants' needs. Manage staff roster and schedules for all home care on daily, weekly, and monthly basis to produce time‑efficient, cost‑effective schedules for participants. Prepare weekly seven‑day home care schedule covering personal care, companion, and respite services. Supervise all personal care assistant staff, including home and center‑based care. Coordinate scheduling for coverage of toileting, bathing, and assign escorts to accompany participants for outings and appointments. Complete human resource management of assigned home care aide staff, including hiring, annual performance appraisals, competency assessments, disciplinary actions, payroll issues, and educational monitoring. Provide routine nursing care for ongoing or episodic illnesses at home according to the plan of care. Provide counseling and education to participants and caregivers regarding physical, emotional and environmental health problems and chronic disease management. Record, maintain, monitor and verify accurate home care records, including service documentation, attendance and payroll, in‑service, medical records and billing according to program policy. Receive and follow up on concerns raised by customers related to home care services. Participate in on‑call coverage rotation with other members of the management team to troubleshoot, advise and coordinate participant care. Obtain and track equipment, supplies and services for home use by participants with assistance of program clerical staff. Ensure routine maintenance of home equipment is completed as required. Work is typically performed in a clinical environment. Accountable for satisfying all job‑specific obligations and complying with all organization policies and procedures. Qualifications Education: Graduate from Specialty Training Program‑Nursing (Required). Experience: Minimum of 1 year in Healthcare (Required). License: Licensed Registered Nurse, State of Pennsylvania – RN.
