Outreach Executive

Oak Street Health seeks an Outreach Executive to connect Medicare-eligible adults in underserved communities with high-quality, preventative primary care. In this role, you will build and maintain relationships with community partners, senior centers, and local organizations to increase awareness of Oak Street Health’s services. You will educate prospective patients on our care model, support them through enrollment, and coordinate first visits. Working in a patient-centered, inclusive, and collaborative culture, you will track outreach activities, meet enrollment goals, and help expand access to care. This is a field-based role with opportunities for growth and meaningful community impact.
