Foster Care LPN: Health Navigator & Immunizations

SCO Family of Services seeks a Licensed Practical Nurse to provide comprehensive health services for children in foster care in New York. Responsibilities include immunizations, assessments, care coordination, and maintaining electronic medical records under NYSDOH guidelines. Role requires LPN licensure in NY, experience with foster care and ability to coordinate with medical providers and caseworkers.
