How You’ll Make a Difference As a Registered Nurse (RN) you will provide one‑on‑one patient care and work at the top of your license. You will be part of an interdisciplinary team that focuses on compassionate, quality care and positive outcomes. You will interact with patients’ families while caring for your patients and experience the privilege of being part of every step of their recovery journey. How You’ll Work You will provide comprehensive skilled care as ordered by the attending physician within the specified geographical territory, working autonomously and efficiently to manage your time. You will be responsible for all practices and duties within the scope of practice outlined by the state. Major Areas of Responsibility Observe and monitor patient conditions; perform OASIS assessments and develop individualized care plans. Administer medication as prescribed by the physician. Prioritize visits for high‑risk patients to decrease rehospitalizations. Manage multidisciplinary care and promote continuity with appropriate admissions, transfers, and discharges. Educate patients and families on disease processes using teach‑back methods. Complete all clinical documentation following agency protocol and Medicare/Federal guidelines. Collaborate with the interdisciplinary team and physicians to ensure optimal care. Follow agency policies, procedures, rules, and regulations; communicate schedule/availability changes to schedulers or supervisors. Perform other duties as assigned. Requirements Registered Nurse with current license in the state of employment. Minimum one‑year experience as an RN in an acute care setting; home‑care experience preferred.
