Key Responsibilities

• Provide skilled physical therapy services to patients in a home health setting across an assigned geographic pod
• Manage and maintain an active caseload, targeting a minimum of 30 points per week to support expected weekly hours
• Proactively backfill schedule gaps caused by patient cancellations by contacting and rescheduling patients to maintain productivity
• Complete clinical documentation accurately and in a timely manner using HCHB (HomeCare HomeBase)
• Collaborate with the home office team to coordinate patient scheduling and caseload management

Required Qualifications

• Active Physical Therapist (PT) license in the state of California
• Minimum 1–2 years of clinical experience as a Physical Therapist
• Home Health experience required
• Proficiency with HCHB (HomeCare HomeBase) documentation platform
• Valid driver's license and reliable transportation for patient home visits

Preferred Experience / Skills

• Familiarity with the Walnut Creek, Lafayette, and surrounding East Bay corridor service areas
• Strong self-management and organizational skills suited to an independent, field-based role
• Experience proactively managing caseload productivity in a home health environment
