Job Overview

General Summary of Position
This role involves assessing and providing physical therapy treatments in line with agency standards, as well as adhering to the laws and regulations that govern physical therapy services in Maryland, the District of Columbia, Virginia, and other relevant regulatory frameworks.

Key Responsibilities
  • Assist in achieving departmental goals and objectives while following established policies, procedures, quality benchmarks, and safety protocols. Ensure compliance with governmental and accreditation standards.
  • Exhibit foundational leadership abilities as a Case Manager and fulfill additional responsibilities as necessary.
  • Act as the primary preceptor and mentor for new nurses and home health aides, overseeing their field training. Collaborate with field trainers to ensure new hires receive the necessary learning experiences to achieve clinical competency. Recommend and provide further education based on identified clinical skill needs.
  • Conduct Joint Observation Visits, validate skills, and participate in mock surveys as required.
  • Demonstrate a comprehensive understanding of Oasis documentation and ensure adherence to regulatory documentation requirements, including care plan development and goal setting, while guiding clinicians in proficient documentation practices.
  • Develop and implement effective physical therapy treatment plans in collaboration with patients, their families or caregivers, physicians, and other healthcare team members, adjusting the plans as necessary. Evaluations will include interviews, observations, palpation, and specialized assessments such as goniometry, muscle strength testing, and sensory testing, utilizing appropriate tools (e.g., goniometer, sphygmomanometer, stethoscope).
  • Independently create and document care plans for patients with varying complexities to ensure continuity of care. Documented interventions should align with the standard of care for the patient's condition and include the use of agency and community resources. Carry out therapy interventions in accordance with home care protocols, demonstrating knowledge of standard supplies and equipment used in patient care.
  • Engage in service recovery efforts by addressing patient and customer concerns, coordinating with interdisciplinary teams, and implementing solutions that foster positive outcomes, patient satisfaction, and operational excellence.
  • Delegate cases to Physical Therapy Assistants (PTAs) as necessary.
  • Initiate and participate in regular case conferences that involve all relevant disciplines, including patients and families in the goal-setting and care plan revision process.
  • Maintain effective relationships with other departments and engage in multidisciplinary quality and service improvement initiatives. Participate in meetings and committees, representing the department and organization in community outreach activities. Enhance personal knowledge and skills through educational programs and affiliations.
  • Conduct patient history and assessments related to episodes of illness to identify habilitation and rehabilitation needs. Recommend adaptive equipment and/or home modifications to optimize patient safety and functional abilities. Identify primary patient issues from assessments and determine discharge planning needs, including referrals to appropriate community resources. Analyze abnormal clinical data to anticipate potential issues related to changing patient conditions.
  • Perform various procedures and modalities, including therapeutic exercises, functional mobility training, gait training, heat/cold applications, and ultrasound.
  • Serve as an educator within the discipline, providing feedback and suggestions to enhance workflow, best practices, and standard procedures. Collaborate with the Education and Operations Team to create a tailored clinical orientation program for new staff, updating it as necessary.
  • Carry out additional duties as assigned.
Minimum Qualifications Education
  • Bachelor's degree in Physical Therapy from an accredited institution is required.
Experience
  • 1-2 years of clinical experience is required.
Licenses and Certifications
  • PT - A valid Physical Therapist License to practice in the District of Columbia, Maryland, or Virginia, depending on work location, is required upon hire.
  • DL - A valid State Driver's License in good standing and a reliable vehicle for transportation is required upon hire.
  • CPR - Certification in Cardiac Pulmonary Resuscitation (including BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross is required within 90 days of hire.
  • Additional unit or specialty certifications may vary by department or business unit.
Knowledge, Skills, and Abilities
  • Strong clinical skills.
  • Excellent organizational abilities.
  • Outstanding verbal and written communication skills.
  • Familiarity with home equipment, home care reimbursement, and regulations governing PTAs.
  • Basic computer proficiency is preferred.
  • Experience with smartphones.

This position offers a salary range of USD $89,065.00 - USD $162,801.00 per year.
