Position Overview As part of the core services team, the PACT RN provides nursing assessment, direct-support services and primary health care to seriously and persistently mentally ill individuals enrolled in the PACT Program. This role provides support and education to other team members in nursing/medical treatment and health care. Essential Job Functions As a team member in the frequent monitoring and assessment of the mental and physical health status of people served With the team in the development of a nursing assessment, comprehensive assessment, and an individualized recovery plan, providing knowledge and expertise from the nursing discipline A population health component for the team: prioritizing medical needs of persons served, tracking medical outcomes over time to determine if improvement occurs, facilitating follow-up for medical conditions when medical intervention is required, and recommending age and gender appropriate screening for persons served (PSA, mammograms, colonoscopy, etc.) In clinical and non-clinical team meetings, including individual and team supervision; actively seeks out monthly supervision from supervisor, and biannual group supervision with the Bridgeway medical APN Provides, Teaches, and/or Monitors Teaching skills to support each person’s individualized recovery journey and community integration Coordinating services with other community mental health and non‑mental health providers, as well as other medical professionals Delivering mental and physical health education, community‑living skill training, and supportive counseling in the home and other natural environments to people served and significant others, often focusing on psychiatric medications, side‑effects and physical health care Assessing service recipients’ stated concerns, mental illness symptoms, and behavior in response to medication, and monitoring for medication side‑effects during observed self‑administration and ongoing face‑to‑face contacts Providing medication education to individuals and family/significant others with service recipient consent Providing 24‑hour on‑call coverage on a rotating schedule, with the ability to respond immediately to individuals by phone or in person Referring to a medical doctor for a physical examination to be performed in the first 3 months of treatment Rapid and flexible response to crisis, including accompanying the person to the local screening center or psychiatric emergency service and remaining with the person during the assessment process Educating the individual regarding his/her illness and the effects and side‑effects of prescribed medications Directing symptom‑management efforts to help each consumer identify symptom and occurrence patterns of his/her mental illness and develop methods to lessen their effects Recognizing high‑risk medical and psychiatric conditions and taking appropriate action Providing technical assistance and education to team members in psychiatric nursing and physical health Conducting metabolic syndrome screening—including obtaining HgbA1c, fasting glucose, lipid profile, and blood pressure Administering prescribed medications and injections, drawing blood, and preparing laboratory specimens for delivery, if certified to do so Training other team members regarding medication education, medication delivery observation of self‑administration, and medication monitoring; and regularly assessing other team members’ competency in this area Maintaining accurate record‑keeping of all medications administered in the Medication Record Adhering to nursing and agency protocols regarding medication administration and storage Consulting with other service providers and resource people regarding assessment, treatment, and rehabilitation needs of the person served Assessing individuals’ daily living skills and teaching needed skills in budgeting, cooking, laundry, cleaning, shopping, personal hygiene, community transportation, self‑advocacy, and ways to maintain housing Using Illness Management and Recovery (IMR) and other evidence‑based practices, which includes stage‑wise treatment utilizing Motivational Interviewing, Cognitive Behavioral Interventions, and teaching strategies Supporting persons served to find and maintain employment and/or education/training Ensuring timely and quality documentation that adheres to DMHAS/Medicaid standards and includes family and/or significant others as permitted by the person served Supporting family and other members of the service recipient's social network to help them manage symptoms and related consequences, reduce family and social stress, and achieve wellness, while providing education to assist the service recipient and family to relate positively and supportively Requirements Education & Certifications Bachelor's degree in nursing preferred Valid NJ Registered Nurse license Experience One year of experience working with individuals with serious and persistent mental illness Driver's License & Vehicle Valid driver’s license required No more than one moving violation within the past 12 months Vehicle required Benefits Medical, Dental, Vision, 403b, basic life and AD&D, flexible spending accounts, EAP Eligible for medical benefits after 30 days of employment Flexible work schedules, clinical training series, leadership development program 10 paid holidays (an 11th after 2 years of employment), generous vacation and sick time Additional Compensation Sign‑on bonus of $5,000 Salary $80,000–$90,000 annually Location 13 Fairfield Avenue, Little Falls, NJ (Serving Passaic County) Work Hours Full time, 40 hours (Monday–Friday 8:00 am–4:30 pm) Bridgeway Behavioral Health Services was founded on a strong desire to fight disparity and injustice alongside people living with behavioral health conditions.
