What You’ll Do: Conduct nutrition assessments, consultations, and follow-up evaluations. Provide individualized nutrition care plans and 1:1 resident consultations. Work with diverse clinical conditions including dialysis, diabetes, cardiac care, wound healing, and tube feeding. Develop and implement individualized nutrition care plans. Monitor nutritional status and recommend appropriate interventions. Collaborate with healthcare professionals to improve nutrition outcomes. Utilize the Nutrition Care Process to deliver high-quality nutrition care. Make a meaningful impact by using Food as Medicine to improve residents' quality of life.
