Case Manager

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal is to enhance the quality of patient management and satisfaction, and to promote continuity of care and cost-effectiveness through the integration of case management, utilization review, and discharge planning.

Responsibilities

• Conduct ongoing assessments of patient needs and develop individualized care plans.
• Implement and evaluate care plans to ensure positive patient outcomes.
• Coordinate with interdisciplinary teams to achieve continuity of care.
• Manage discharge planning processes effectively.
• Comply with regulatory and departmental guidelines and policies.
• Maintain detailed documentation and communication regarding patient care.

Qualifications

• Minimum 4 years of experience as a Registered Nurse required.
• Graduate from an accredited nursing school; BSN preferred.
• Two years of health care experience required.
• Recent acute hospital experience is required.
• EPIC experience is required.
• Active license to practice as a Registered Nurse in North Carolina.
• BLS certification required; ACM/CCM certification preferred.
• No first-time travelers or locals within a 50-mile radius will be considered.

Work Schedule

• Shift: Monday to Friday, 08:30 AM - 05:00 PM (5x8-Hour Shift).
• RTO (Requested Time Off) is allowed, with specific blackout dates noted.

Additional Information

• Average daily census is approximately 20 patients during the day shift and 70 on weekends.
• Orientation for the position consists of 24 hours.
• Candidate must be adaptable to frequent changes and should demonstrate strong organizational skills.
