Shelby Skilled Nursing - Sidney, Ohio

Job Summary:

We are seeking a compassionate and skilled Social Services/Activity Director to join our team at Shelby Skilled Nursing and Rehabilitation. As an integral member of our leadership team, you will be responsible for developing and maintaining relationships with residents, family members, and caregivers, ensuring smooth transitions into care, and managing relationships with referral partners. Our 50-bed skilled nursing facility in the heart of Shelby, Ohio, is dedicated to serving our residents and families with excellence. As a key member of our team, you will play a vital role in welcoming new residents, guiding families, and showing our community the warmth and quality of life that Shelby offers.

Responsibilities:

  • Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences

  • Identify and address any issues or concerns related to our activity program or residents' care

  • Participate in interdisciplinary team meetings to share information and collaborate on resident care

  • Stay up-to-date with industry trends and best practices in activity programming and social work

  • Complete monthly activity calendars

  • Help assist resident and family with a safe discharge

  • Represent Shelby Skilled Nursing and Rehabilitation at community events and conferences

Requirements:

  • Develop and maintain relationships with residents, family members, and caregivers to understand their needs and preferences

  • Identify and address any issues or concerns related to our activity program or residents' care

  • Participate in interdisciplinary team meetings to share information and collaborate on resident care

  • Stay up-to-date with industry trends and best practices in activity programming and social work

  • Represent Shelby Skilled Nursing and Rehabilitation at community events and conferences

Job Type:

  • Full Time

  • Day Shift

  • Occasional evenings or weekends for facility events

  • Manager on Duty (some weekends)
