Employment Type: Full-Time

Schedule: M-F, 35 hours

Salary: $23.00/hr.

Qualifications:

Education: Associate degree in Human Services, Social Work, Psychology, Counseling, Sociology, Criminal Justice, or a related field preferred .

Experience: Minimum one (1) year of experience working with individuals experiencing homelessness, substance use disorders, domestic violence, mental health conditions, or other vulnerable populations.

Relevant years of experience and education will be considered

About the role:

Under the supervision of the Residential Director, the Case Manager/Housing Counselor provides housing stabilization and supportive case management services to individuals and families participating in the ESSHI Permanent Supportive Housing Program. Utilizing a Housing First, trauma-informed, and recovery-oriented approach, the Case Manager/Housing Counselor assists residents in maintaining stable housing, achieving individualized goals, accessing community resources, and increasing self-sufficiency. The position is responsible for assessments, service planning, resource coordination, advocacy, documentation, and supportive service delivery in accordance with OASAS Permanent Supportive Housing Guidelines, ESSHI requirements, and agency standards.

Key Responsibilities

· Conduct housing pre-screenings and eligibility reviews.

· Complete intake assessments upon admission.

· Identify participant strengths, needs, barriers, supports, and service priorities.

· Participate in admissions and orientation activities.

· Assist with development of initial housing stabilization strategies.

· Develop individualized Service Plans within 30 days of program admission.

· Establish measurable goals with participants related to housing stability, wellness, recovery, employment, education, benefits, and self-sufficiency.

· Complete Service Plan reviews and updates quarterly.

· Complete annual Service Plan revisions.

· Help participants learn and maintain independent living skills.

· Assist participants with apartment upkeep and housing responsibilities.

· Support development of budgeting and financial management skills.

· Assist with menu planning, nutrition education, and daily living skills development.

· Promote long-term housing retention and stability.

· Assist participants in obtaining and maintaining public benefits.

· Coordinate applications and recertifications for SNAP, Medicaid, SSI, SSDI, Public Assistance, and other entitlement programs.

· Connect participants with healthcare, behavioral health, substance use treatment, employment, education, and vocational services.

· Coordinate with Health Homes and Care Managers.

· Advocate for participant needs with community agencies and service providers.

· Facilitate referrals and linkages to supportive services.

· Coordinate care with treatment providers and community partners.

· Participate in case conferences and multidisciplinary meetings.

· Conduct home visits based on participant needs and program requirements.

· Monitor participant progress toward Service Plan goals.

· Assesses housing stability and emerging needs.

· Address barriers impacting housing retention.

· Maintain resident charts and service documentation.

· Complete case notes documenting all participant interactions.

· Document home visits, referrals, assessments, outcomes, and follow-up activities.

· Maintain releases of information and consent forms.

· Ensure documentation complies with OASAS, ESSHI, agency, and funding requirements.

· Participate in outreach and recruitment efforts.

· Assist with community presentations and referral development.

· Participate in staff meetings, supervision, and required training activities.

· Complete discharge planning and transition support.

· Coordinate referrals for continuing services.

· Complete discharge summaries and required closing documentation.

· Other duties as assigned.

Compliance: Adheres to all applicable federal, state, local, funder, and agency laws, regulations, policies, and procedures, including those governing confidentiality, privacy, documentation, client records, mandated reporting, incident reporting, and participant rights. Maintains accurate and timely case records, service plans, progress notes, home visit documentation, releases of information, and other required documentation in accordance with OASAS, ESSHI, and agency standards. Reports incidents, safety concerns, abuse allegations, hospitalizations, deaths, and other reportable events in accordance with agency, ESSHI, OASAS, Justice Center, and funder requirements. Completes required training and participates in supervision, quality assurance activities, program monitoring, and corrective action initiatives as assigned.
