Case Manager

To support patients navigating complex diagnoses, the full-time California Licensed Case Manager RN will assess patient needs, develop individualized treatment plans, and maintain ongoing communication with care teams while working in a remote environment. Key responsibilities Conduct thorough assessments of patients' physical, psychosocial, emotional, and financial needs Develop and monitor individualized treatment plans in collaboration with patients and care teams Advocate for patients by facilitating access to quality care and wellness resources Required qualifications Graduate of an accredited Registered Nursing (RN) program Current California RN license required; multi-state license also required Minimum of 5 years medical/surgical or acute care experience, including 2 years in case management Knowledge of medical claims processing and relevant coding (ICD-10, CPT, HCPCS) Proficiency with Microsoft Office Suite and case management platforms such as Eldorado and UM Web
