Physical Therapist – Total Joint (Ambulatory Surgical Center & Home PT) Compensation Pay or shift range: $90,000 USD to $110,000 USD. The estimated range is the budgeted amount for this position. Description JAG Physical Therapy is seeking a highly motivated, orthopedic‑focused Physical Therapist to join our Total Joint Program, a unique hybrid role supporting patients across the full surgical continuum of care — from Ambulatory Surgical Center (ASC) to post‑operative Home Physical Therapy. This clinician will work directly with total hip and total knee replacement patients, delivering structured education, early mobility training, and post‑operative rehabilitation. Licensed Physical Therapist in the state practicing in ___ (state name). Completed state‑regulated CEU requirement in the appropriate state (NY, NJ). Experience/knowledge with an EMR software is preferred. Excellent oral and written communication. Strong clinical decision‑making skills. Excellent work ethic and dedication to patient success. Basic computer skills. Works as a team member and individually with minimal supervision. Maintains a positive, professional demeanor at all times. Responsibilities Collects and records patient care data following format and guidelines of the patient care service, regulatory agencies, and third‑party payers, reflecting an understanding of reimbursement systems. Creates and updates daily billing records, following department guidelines for productivity. Prepares for evaluations in accordance with department policies and standards of practice, obtaining patient history and researching unfamiliar diagnoses. Selects and administers tests and measures. Synthesizes evaluation results to formulate diagnosis, goals, prognosis, and plan of care. Establishes treatment plans based on evaluation results and best practice, collaborating with interdisciplinary team, patients, and patient support systems. Assesses and re‑assesses need for modifications to plan of care and goals. Identifies need for consultations or referrals to other health care professionals or services and initiates discharge plans. Provides education/training to patients and support systems, including ADLs/IADLs, therapeutic activities (e.g., transfers, bed mobility), rehabilitation equipment prescription, and community reintegration. Responsible for treatment, plan of care, and exercise prescription for patient caseload. Provides interventions that are safe, effective, and in compliance with JAG guidelines. Performs musculoskeletal interventions such as therapeutic exercise, joint mobilization, and modalities. Provides data and note writing using Prompt EMR for all treatments. Attends annual JAG Billing and Coding seminar. Promotes JAG in the community through lectures and presentations. This job description includes the essential functions and general nature of the work.
