Pharmacist - Clinical Anticoagulation Management Services

We are seeking an AMS Pharmacist to lead anticoagulation management, ensuring patient safety and optimal outcomes. In this role, you will manage warfarin therapy, provide medication therapy management , and coordinate care transitions . You will educate patients and serve as a consultant for healthcare professionals . Responsibilities include supervising staff, developing clinical guidelines, ensuring compliance, and managing drug interactions. You will triage INR reviews, monitor high-volume patient encounters , and contribute to clinical safety and protocols . Qualifications require a Doctor of Pharmacy (PharmD) or equivalent with licensure as a Registered Pharmacist (RPh) . Experience in anticoagulation management and strong knowledge of clinical guidelines are preferred. Key skills include clinical expertise, leadership, communication, and collaboration.
