Physical Therapist (PT) – Skilled Nursing Facility Full-Time | $38-$55/hr - depending on experience Job Overview We are seeking a compassionate and dedicated Physical Therapist (PT) to join our rehabilitation team within a Skilled Nursing Facility (SNF) setting. The Physical Therapist will work closely with residents recovering from illness, injury, or surgery to restore mobility, improve strength and balance, and help patients achieve the highest possible level of independence. This role is ideal for therapists who enjoy working with the geriatric population and collaborating with an interdisciplinary care team. Responsibilities Evaluate residents and develop individualized physical therapy treatment plans Provide therapeutic interventions including strength training, balance training, gait training, and mobility exercises Assist residents in improving functional mobility, transfers, and ambulation Collaborate with interdisciplinary care teams including OT, SLP, nursing staff, and physicians Educate residents and caregivers on safety, fall prevention, and home exercise programs Participate in care planning meetings and discharge planning Qualifications Current Iowa Physical Therapist license Graduate of an accredited Physical Therapy program recognized by the Commission on Accreditation in Physical Therapy Education (CAPTE) Eligible for or holding certification through the Federation of State Boards of Physical Therapy Strong communication and documentation Physical Requirements Ability to assist patients with mobility and transfers Frequent standing, bending, reaching, and walking Ability to lift up to 50 lbs with assistance as needed Competitive salary- depending on experience and certifications Medical, dental, and vision insurance Additional voluntary benefits Realistic productivity expectations Supportive team environment Equal Opportunity Employer, including disabled and veterans. #J-18808-Ljbffr Career Links
