Home Health Registered Nurse (RN) - Littleton, Colorado

An opportunity is available for an experienced and compassionate Home Health Registered Nurse to deliver skilled nursing care in a field-based, patient-centered setting. This contract position centers around providing individualized, high-quality care to patients in their homes, supporting continuity, compliance, and clinical excellence. Candidates must be organized, self-driven, and comfortable operating independently while effectively collaborating within interdisciplinary teams.

Schedule/Hours:

• Full-time 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Expect 25-32 points per week (approximately 35 patient visits daily)
• Field-based, with daily travel within your assigned area

Key Responsibilities:

• Complete comprehensive OASIS assessments and devise tailored care plans
• Deliver skilled nursing services like wound care, IV therapy, medication administration, and chronic disease management
• Monitor patient progress, promptly communicating changes to the care team and physicians
• Educate patients and families on their diagnoses, care plans, medications, and health strategies
• Maintain accurate and timely documentation using Home Care Home Base (HCHB)
• Work closely with physical therapists, occupational therapists, medical social workers, and aides to ensure cohesive care delivery
• Participate in staff meetings, case conferences, and continuing education as required
• Consistently adhere to regulatory, documentation, and agency care standards

Qualifications:

• Active RN license in good standing
• At least one year of recent home health experience (field-based preferred)
• Proficient in Home Care Home Base (HCHB) and OASIS assessments
• Strong assessment, clinical, and critical thinking abilities
• Reliable transportation, valid drivers license, and auto insurance
• Independent, self-organized work habits and excellent communication skills
• BSN and experience managing 25-32 points per week preferred
• Familiarity with Medicare and home health compliance is a plus

$1,954 - $2,067/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
