Pharmacist

Pharmacist Menomonee Falls, WI

Pay From: $61.00 per hour

MUST: Experienced pharmacist with 5 plus years of experience as a pharmacist. Must have an active Wisconsin pharmacist license. Must have inpatient experience. Must have PIC (Pharmacist in Charge) experience.

DUTIES:

Pharmacist will be responsible for, but not limited to the following:

• Review all patient charts for correct medication therapy and documentation; including patient drug allergies, drug/drug interactions with current medications, appropriate medication choice, route of administration and dose
• Conduct weekly on-site visits to each facility
• Conduct monthly unit review of department
• Assist Hospital Administrator to ensure sufficient medication is in stock; including working with the pharmaceutical wholesaler to find alternatives, if necessary
• Review and reconcile controlled substances logs and conduct an annual controlled substance inventory
• Assist clinical staff with drug related information
• Counsel patients on medication therapy, if needed
• Plan, monitor and evaluate drug programs and regimens
