Responsibilities Include:

• Performing skilled nursing visits in the home setting
• Developing and following individualized care plans
• Monitoring patient progress and communicating updates
• Providing medication management and treatments
• Educating families on disease management and care techniques
• Completing accurate and compliant charting

We’re Looking For:

• An Arizona-licensed RN in good standing
• Dependable transportation and current driver’s license
• Strong clinical judgment and communication skills
• A passion for helping patients maintain independence at home
