RN Case Manager for Home Health

RN Case Manager for home health. Must have case management experience. OASIS experience is required. Wound care experience is preferred. Candidates should be coming from a high-volume home health role. The position requires seeing 5-6 patients per day. There will be a high volume of documentation involved.
