Position Relief/Per Diem Assistant Case Manager Location: Westwood, MA Shift: Varies. This posting is for Per Diem only. Essential Duties Provide individual and small group instruction in self‑care, wellness, activities of daily living, communication, social and motor skills. Provide direct care for ADLs and group activities; lift and transfer individuals as appropriate to their needs. Communicate respectfully and pro‑actively with families, residential staff and all members of participants’ teams. Attend in‑service trainings as required. Comply with all applicable State and Federal regulations, agency policy and procedures, and personnel practices. Protect the health and safety of all participants and staff. Excellent interpersonal skills. Ability to work in a team environment. Perform other duties as requested by supervisors. Requirements Experience working with individuals with ID/DD. High school diploma/GED, related experience preferred. Must be able to pass fingerprint background check, CORI, DPPC and driving background checks. Valid driver’s license required.
