In-Home Health - Physician (Full Time) - Hartford CT

Overview Signify Health, part of CVS Health®, is seeking a Full‑time Physician to provide In‑Home Health Evaluations. The role involves engaging with patients in the comfort of their own homes, conducting physical exams and reviewing medication and medical histories, and coordinating additional diagnostic tests when ordered by the patient’s health plan. Responsibilities Bring your heart into every visit, building trusted relationships to improve health outcomes. Participate in a national network of clinicians dedicated to connecting patients with appropriate care. Work with Signify Health as a clinician colleague, fulfilling the requirements of the specified role. Be flexible to travel locally and within licensed states, as needed. Conduct virtual visits as required. Provide all Signify Health services as indicated, including Diagnostic & Preventive Services products (e.g., spirometry, DEE, etc.). Work Environment Your primary workplace is the private residence of health plan members. Candidates must be able to safely and comfortably work in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants. Base pay per In‑Home Health Evaluation ranges from $145.00 – $145.00 per hour. Additional reimbursement may be available for completion of Diagnostic and Preventive Services (DPS) as ordered.
