Occupational Therapist: Home Health

Position

Full Time Occupational Therapist

Anticipated Territory: Antioch and surrounding cities*

Location note: All opportunities require being in the field visiting patients in their homes; remote/work-from-home positions are not available unless specifically noted.

Responsibilities

• Provide skilled occupational therapy services in patients’ homes in partnership with the Case Manager to evaluate and address needs so patients can live safely and productively at home.
• Address areas such as home safety, medication management, wheelchair seating and positioning, pain management, falls prevention, chronic disease management, and dementia family caregiver training.
• Serve as an educator, patient advocate, and member of the interdisciplinary care team.

Qualifications

• Current unrestricted CA Occupational Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of local community resources
• Knowledge of development, behavior in relation to illness/injury, and cultural influences on health care
• Knowledge of state and federal homecare regulatory guidelines (preferred)
• Experience in home health care (highly preferred)
• Experience with high-acuity patients (a plus)
• Experience working with an interdisciplinary team (highly desired)
• Case Management fundamentals (highly desired)
• Self-directed, empathetic, critical thinker with outstanding interpersonal skills, leadership, and organizational abilities

Requirements to be a successful candidate

• Before scheduling an in-person interview, completion of a computer literacy assessment to ensure proficiency with electronic charting and web-based communication tools (assessment results used as part of the hiring process).

Compensation and Benefits

$120,000 - $130,000 per year; starting pay commensurate with experience. 21HHS offers generous bonus incentive plans and comprehensive health benefits including 401k with match.
