How you’ll contribute Build and maintain referral relationships to drive appropriate admissions to the acute rehab unit. Assist and coordinate the intake and pre‑admission screening process. Perform on‑site clinical judgment to determine patient appropriateness for admission. Communicate rehabilitation and facility options to patients and families and assess expectations and participation requirements. Secure information relating to patients’ resources and benefits. Act as initial contact for external case management and discharge planner personnel with payors and hospital facilities. Spend around 80% of the day in assigned territory. Previous clinical experience required, with demonstrated skills in clinical assessment and EMR systems. Valid driver’s license. Previous marketing/sales experience preferred.
