• Develop and implement individualized care plans, in collaboration with other members of the healthcare team
• Participate in recognizing and addressing residents' physical, emotional, and social needs, and provide interventions to promote overall wellness
• Communicate effectively with residents, families, and other healthcare professionals to ensure comprehensive care
• Participate in ongoing training and education to stay current with the latest industry developments and best practices
