Monitor residents’ health status, document vital signs, and promptly notify the care team of changes. Perform wound care, routine procedures, and assist with peri‑care and injections as ordered. Assist residents with activities of daily living and mobility; support safe transfers and use of assistive devices. Collaborate with the interdisciplinary team; communicate care plans and progress to colleagues and families. Maintain accurate documentation in the electronic health record and ensure compliance with infection control and safety protocols. Requirements Active Licensed Practical Nurse license (state‑issued; eligible to practice in the assignment state). Current CPR/BLS certification. Ability to work 12‑hour shifts in a long‑term care setting. Strong communication skills and ability to collaborate with an interdisciplinary team. Prior long‑term care experience or familiarity with LTC care plans preferred. Ability to meet health, safety, and vaccination requirements as required for the assignment.
