Key Responsibilities

• Provide daily personal care and assist residents with activities of daily living (ADLs), including bathing, grooming, shaving, mouth care, foot care, and male/female perineal care.
• Assist residents with ambulation, feeding, showering, toileting, and urinary/stool incontinence care.
• Perform complete bed baths, bed shampoo, and occupied/unoccupied bed linen changes.
• Assist with turning and repositioning residents and implement pressure injury prevention measures, including heel and elbow protection.
• Support safe patient transfers using Hoyer lifts, transfer belts, wheelchairs, stretchers, and bedside commodes.
• Respond promptly to call lights and assist residents with bedpans, urinals, and specialty beds.
• Measure, record, and report resident weights, urine output, drain output, dietary intake, and calorie counts.
• Collect urine specimens, label and transport specimens, and maintain accurate patient care documentation.
• Follow fluid restrictions, isolation precautions, PPE protocols, and infection control procedures.
• Monitor and report changes in residents' conditions to the nursing team.
• Clean patient care equipment and assist with oxygen tanks, anti-embolic stockings, and specialty air mattresses as needed.
• Maintain resident belongings and ensure a safe, clean, and comfortable care environment.
