Job Summary Baptist Health Medical Group | Dietitian - Diabetes Educator | Endocrinology | New Albany, Indiana | Part Time, First Shift | $5,000 Sign-On Bonus Responsibilities Provide nutritional assessment, education, and management to patients with chronic conditions who are at risk for disease progression, complications, cognitive, and functional decline. Collaborate with physicians and the interdisciplinary team to develop and implement individualized care plans that promote patient understanding and management of their condition, optimize quality of life, and improve clinical outcomes. Develop various nutrition programs (adult and pediatric focused) for the Joslin Diabetes Center. Provide service in the Endocrinology office and remotely as appropriate. Qualifications Bachelor’s degree with coursework approved by the Academy of Nutrition and Dietetics Accreditation Council for Education in Nutrition and Dietetics (ACEND). Certified Diabetes Educator certification and passed the national examination administered by the Commission on Dietetic Registration. Current, unrestricted certification with the Commission on Dietetics Registration (CDR). Current, unrestricted dietitian licensure in Kentucky and Indiana. Knowledge of nutrition guidelines in chronic disease management. Knowledge of medical terminology. Knowledge of HEDIS or NCQA quality measures. Valid unrestricted driver’s license and ability to drive to multiple locations.
