Homeside Rehab is seeking compassionate and dedicated Speech-Language Pathologists (SLPs) to provide individualized, evidence-based speech therapy services to patients in their homes throughout the Hudson Valley, New York .

Responsibilities

• Perform comprehensive speech-language evaluations and develop individualized treatment plans based on each patient's needs and goals.

• Provide evidence-based treatment for speech, language, articulation, fluency, voice, cognitive-communication, and swallowing disorders, when applicable.

• Deliver skilled speech therapy services in patients' homes while promoting improved communication and functional independence.

• Complete accurate, timely, and compliant electronic documentation.

• Collaborate with physicians, caregivers, and the interdisciplinary healthcare team to coordinate patient care.

• Educate patients and caregivers on home programs and communication strategies.

• Communicate effectively with office staff regarding scheduling, patient progress, and clinical needs.

Qualifications

• Active New York State Speech-Language Pathologist License (Required)

• Minimum 1 year of experience in Home Health, Outpatient, Early Intervention, or Skilled Nursing Facility (SNF) preferred

• Excellent clinical assessment and communication skills

• Ability to work independently in a home health setting

• Basic computer proficiency for electronic documentation

• Valid New York State driver's license and reliable transportation preferred

Position Details

• Job Types: Full-time, Part-time, Per Diem

• Compensation: Competitive hourly compensation based on experience

Schedule

• Choose your own hours

• Monday through Friday

• Day and evening shifts available

• Weekend availability optional
