As a valued member of our team, you will have the opportunity to make a real difference in the lives of our residents and their families.Job SummaryWe are seeking an experienced and compassionate Licensed Vocational Nurse (LVN) to join our team at a Skilled Nursing Facility. As an LVN, you will be responsible for providing direct nursing care to our residents under the direction of a Registered Nurse (RN), and ensuring that the facility meets all applicable regulations and standards.NOC shift (11PM-7AM) is available at this time. Experience with med pass and Point Click Care is required.Responsibilities:Provide direct nursing care to residents, including administering medications and treatments, monitoring vital signs, and assessing and documenting changes in their conditionAssist with the development and implementation of individualized care plans based on residents' needs and goalsCoordinate care with other members of the healthcare team, including physicians, therapists, and other healthcare professionalsEnsure that all documentation and charting is accurate, complete, and up-to-dateAssist with activities of daily living (ADLs) as neededEducate residents and families about their healthcare needs and treatmentsRespond to emergencies and provide first aid as neededAdhere to all policies, procedures, and regulations governing nursing care in the facilityQualifications:• Valid and current LVN license in the state of California• Graduate of an accredited nursing program• Minimum of 1 year experience in a skilled nursing facility, acute care setting, or in-patient rehabilitation or in-patient behavioral health setting• Strong clinical skills, including assessment, planning, and implementation of nursing care• Excellent communication and interpersonal skills• Ability to work independently and as part of a team• Strong critical thinking and problem-solving skillsWhat We Offer:• Competitive wages: $34 - $38 HOURLY• Opportunities for professional growth and advancement within our organization• Commitment to providing high-quality care to our residents• Collaborative and supportive work environmentIf you are a caring and dedicated LVN looking for a rewarding career in a supportive and collaborative environment, we encourage you to apply for this position.
