Job Summary

We are seeking a dedicated and skilled Physical Therapist to join our healthcare team. The ideal candidate will provide comprehensive patient care to patients on an intermittent basis in their place of residence. This role involves developing and implementing individualized care plans to improve patient mobility, function, and quality of life. The Physical Therapist will utilize their expertise in physiology, anatomy, and medical terminology to deliver evidence-based physiotherapy services tailored to diverse patient populations such as geriatrics and those recovering from acute care or hospital stays. Duties

• Assess patient needs through detailed evaluations of physical condition and medical history
• Develop personalized care plans incorporating therapeutic exercises, manual therapy, trigger point therapy, and other modalities
• Implement treatment sessions focused on improving mobility, strength, and functional independence
• Manage discharge planning to ensure seamless transition from hospital or inpatient settings to home
• Monitor patient progress and adjust treatment strategies accordingly
• Educate patients and caregivers on exercises, injury prevention, and health maintenance techniques
• Document all patient interactions accurately using EMR systems in compliance with healthcare regulations
• Collaborate with multidisciplinary teams including physicians, nurses, and other healthcare professionals to optimize patient outcomes
• Participate in ongoing professional development to stay current with advances in physiotherapy practices

Qualifications

Qualifications

1. Graduate from a physical therapist curriculum approved by the American Physical Therapy Association, or the Committee on Allied Health Education and Accreditation of the American Medical Association, or the Council of Medical Education of the American Medical Association and American Physical Therapy Association.
2. Valid licensure as a licensed Physical Therapist in the state of practice
3. Proven experience in acute care, inpatient, outpatient, hospital, or home health settings
4. Strong knowledge of physiology, anatomy, and medical terminology
5. Experience with care plans, discharge planning, and managing diverse patient populations
6. Familiarity with EMR systems for documentation and patient record management
7. Skills in manual therapy techniques such as trigger point therapy and specialized treatments for orthopedics
8. Excellent communication skills for patient education and interdisciplinary collaboration
9. Ability to work independently while maintaining a team-oriented approach
