Key Responsibilities

• Provide skilled physical therapy services to a geriatric home health patient population
• Conduct and complete OASIS assessments , including start of care evaluations
• Manage a caseload of up to 6 patient visits per day, meeting a standard of 30 productivity points
• Document all clinical activity accurately and timely using HCHB (HomeCare HomeBase) EMR
• Utilize personal transportation to travel to patient homes; service area may occasionally extend up to 50+ miles

Required Qualifications

• Active California Physical Therapist (PT) license required
• Basic Cardiac Life Support (BCLS) certification required
• Minimum of 1 year of recent skilled home health experience (at least 1 year within the last 3 years)
• Proficiency with HCHB EMR required
• OASIS experience required
• Must have reliable personal transportation

Preferred Experience / Skills

• Experience working with geriatric patient populations in a home health setting
• Strong time management skills with the ability to efficiently manage a multi-patient daily caseload
