Oasis Therapy Services is seeking a Physical Therapist (PT) to provide one-on-one rehabilitation services in an assisted living and memory care setting in Beaver Dam, Wisconsin. Available employment options: PRN EMR: Net Health Responsibilities Evaluate mobility, strength, balance, gait, and fall risk. Develop and provide individualized treatment plans to improve safe mobility and independence. Monitor progress and adjust treatment plans as needed. Educate residents and caregivers on exercises, transfers, fall prevention, and assistive devices. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Wisconsin Physical Therapist license. Graduate of an accredited Physical Therapy program. Current CPR certification. Strong communication, organization, and documentation skills. Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred. Why Join Oasis?
