Physical Therapist - Travel Contract

Looking for a change of scenery and a chance to help others. Join as a traveling Physical Therapist providing home health care in Logansport, IN. You will help patients recover from injuries and manage chronic conditions while working across local coverage areas. You will complete evaluations, develop plans of care, perform OASIS visits, and document care within required timeframes.

Minimum Requirements

• Doctoral, Master’s, or Bachelor’s Degree in Physical Therapy from an accredited program.
• Current, unencumbered Indiana Physical Therapist license prior to start.
• Recent home health experience required.
• Proficiency with HomeCareHomeBase or similar home health documentation systems.
• Understanding of Medicare guidelines for home care.
• CPR certification.
• Valid driver’s license, reliable vehicle, and vehicle insurance.
• Ability to complete documentation within 24 hours of visit and meet productivity expectations.

Preferred Qualifications

• Experience supervising PTA care per company policy and state regulation.
• Teamwork and strong communication skills.

Assignment Details

• Shift hours: Monday–Friday, 8:00 AM–4:30 PM.
• Weekly hour guarantee: 36 hours. Productivity expectation: 30 points per week.
• Coverage areas include White, Pulaski, Fulton, Cass, Howard, Carroll, Miami, and Tipton counties.
• Mileage reimbursed between visits from first patient to last patient.
• No on-call requirement.
• Schedule flexibility required, including participation in weekend rotation and potential availability on observed holidays.

Impacting Patient Care

Complete patient evaluations and develop individualized PT plans of care within state-specific guidelines. Perform Start of Care, Resumption of Care, evaluations, and discharges per home health standards. Participate in branch meetings and case conferences as required by leadership. Provide schedule flexibility to meet patient needs and ensure continuity of care.
