Responsibilities
- Under the direction of the RN, assist in identifying the physical and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations
- Participate in planning and implementing care in conjunction with the RN
- Provide care utilizing infection control measures that protect both the staff and patient
- Assure continuity of quality patient care delivered with appropriate documentation
- Prepare regular clinical/progress notes and summaries
- Assist the patient with the activities of daily living while teaching appropriate self-care techniques
- Provide and maintain a safe environment for the patient, including an optimum physical and emotional environment
- Maintain client confidentiality
- Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury, or significant change in the patient's condition
Qualifications
- Graduate of an accredited school of vocational nursing
- Proof of current LVN license
- Proof of current CPR certification
- Ability to work flexible schedule
- Minimum 1 year experience as an LVN in a clinical setting preferred
- Excellent interpersonal skills
- Bilingual preferred
Valid driver’s license and reliable transportation
