Licensed Practical Nurse (LPN) - Hospice

Key Responsibilities Provide skilled hospice nursing care under the direction of a Registered Nurse and the established plan of care Administer medications and treatments in accordance with physician orders and agency policies Monitor and document changes in patients' physical, emotional, and psychosocial status, reporting significant findings promptly Educate patients and caregivers on comfort measures, medications, and what to expect throughout the hospice journey Collaborate closely with RNs, social workers, aides, chaplains, and other team members to ensure coordinated, patient-centered care Maintain accurate, timely clinical documentation in the EMR in compliance with CareM standards Participate in interdisciplinary team meetings, case conferences, and quality improvement activities Navigate complex family dynamics with professionalism, empathy, and healthy boundaries Requirements What We're Looking For Graduate of an accredited practical or vocational nursing program Current, unrestricted LPN license in the state of New Mexico Prior hospice experience strongly preferred Minimum of one year of clinical nursing experience (home health, hospice, or acute care preferred) Current CPR (BLS) certification Strong clinical judgment, time management, and communication skills Ability to work independently in the field while staying closely connected to the care team Emotional steadiness, compassion, and reliability - especially in difficult situations Valid driver's license, reliable transportation, and current auto insurance Why Join Us Supportive leadership and strong RN partnership - you're never on your own Manageable caseloads with realistic expectations and time to build relationships Autonomy in the field, paired with responsive clinical and operational support Comprehensive benefits package, including medical, dental, vision, PTO, and holidays Mileage reimbursement and the tools you need to succeed in the field Ongoing training, education, and opportunities to grow within hospice care A mission-driven culture grounded in trust, integrity, collaboration, and compassion At Corus Health, our mission is to bring out the courage in others - in our patients, their families, and each other.
