Home Health Registered Nurse (RN) Case Manager Service Locations: Waukesha County and Lake Country Employment Type: Full-Time (Exempt) Pay Rage: $80,000-$85,000 per year Position Summary Here at BrightStar Care of Greater Waukesha and Lake Country , we are seeking a compassionate and skilled Home Health Registered Nurse (RN) Case Manager to join our home health team. In this role, you will provide high‑quality skilled nursing care, coordinate services, and support patients in achieving their highest level of independence at home. If you are a clinically strong RN who thrives in a field‑based environment and values meaningful patient relationships, this is an excellent opportunity. Perform comprehensive initial and ongoing patient assessments in the home setting. Develop individualized care plans based on patient needs, physician orders, and regulatory requirements. Deliver skilled nursing interventions including wound care, medication management, IV therapy, catheter care, and disease‑specific education. Monitor patient progress, identify changes in condition, and communicate updates promptly to physicians and interdisciplinary team members. Coordinate services with nurses and caregivers to ensure continuity of care. Complete timely, accurate documentation including assessments and visit notes. Educate patients and caregivers on disease processes, medications, safety, and self‑management strategies. Participate in case conferences, quality initiatives, and on‑call rotation as assigned. Current and unrestricted Registered Nurse (RN) license in the state of practice. Five (5) years of clinical experience (Preferred) Two (2) years of experience in a home health agency (Preferred) Strong assessment, critical‑thinking, and communication skills. Ability to work independently, manage a field‑based schedule, and prioritize multiple tasks. Knowledge of Medicare home health regulations and documentation standards preferred. Valid driver's license and reliable transportation.
