Full-time Home Health PT, Physical Therapist

Key Responsibilities Conduct thorough physical assessments and review medical histories to evaluate patients' conditions. Develop and customize treatment plans tailored to each patient's specific needs and objectives. Implement a range of therapeutic exercises and training programs to enhance patient recovery. Provide education to patients and their families on injury prevention and wellness strategies. Monitor and document patients' progress towards achieving their rehabilitation goals. Collaborate with other healthcare providers to ensure comprehensive and holistic patient care. Maintain accurate and up-to-date patient records in compliance with regulatory standards. Qualifications Master’s degree in Physical Therapy. Minimum of 1 year of experience in home health as a physical therapist. Current licensure to practice as a physical therapist in the state of operation, adhering to professional standards. Valid CPR certification required throughout employment in a clinical role. Must possess a valid driver's license and the ability to travel to various business locations.
