Work Location: On the Road – East Valley, Arizona (Chandler, Mesa, Gilbert, Tempe, Queen Creek, Apache Junction, and Scottsdale)
We are seeking a dedicated Licensed Practical Nurse (LPN) to provide skilled nursing care to patients in their homes under the supervision of a Registered Nurse.
In this role, you will deliver high-quality, patient-centered care including wound care, medication administration, monitoring patient conditions, and completing accurate and timely documentation in accordance with care plans and agency policies. Pay are per visit.
Job Type: PRN/as needed
Responsibilities:- Provide skilled nursing care in patients’ homes
- Perform wound care and medication administration
- Monitor and report changes in patient conditions
- Complete required clinical documentation accurately and on time
- Collaborate with the RN and care team to ensure quality patient outcomes Qualifications:
- Active LPN License (required)
- Current BLS Certification
- Current CPR Certification
- Minimum 1 year of nursing experience
- Home health experience preferred
