Job Description
Title: Physical Therapist

Schedule: 5 days/week, 40 hours/week

Assignment Details:
- Guaranteed Hours: 40
- Contract Length: 13 weeks

Requirements:
- Licensed Physical Therapist in the state
- Current driver's license required and ability to travel to patients' homes
- Strongly prefer 2 years previous experience as a Physical Therapist

Roles & Responsibilities:
- Perform comprehensive physical therapy assessments and develop individualized, evidence-based treatment plans.
- Supervise and coordinate care with Physical Therapist Assistants.
- Provide therapeutic interventions focused on mobility, strength, pain reduction, and functional independence.
- Collaborate with interdisciplinary team members to coordinate patient-centered care and discharge planning.
- Educate patients and caregivers on safe mobility techniques, equipment use, and home-exercise programs.
