Compassionate Hospice Home Care RN Case Manager
Workstream
Workstream is hiring a registered nurse in Abilene, Texas, to coordinate home care services and support community health. You will develop care plans, educate patients and caregivers, and oversee home health aide care, with a strong emphasis on effective communication and patient outcomes. A qualified nurse will have Texas licensure, a minimum of two years of experience, and a commitment to patient-focused care.
