Family Practice/Primary Care Nurse Practitioner

We are seeking a Family Nurse Practitioner who is passionate about improving health outcomes and making a meaningful impact in the community.

Schedule

• Full-Time
• Monday-Friday
• 7:00 AM to 4:00 PM
• No evenings, weekends, or major holiday clinic hours

Responsibilities

• Provide primary care services for patients of all ages
• Diagnose and treat acute and chronic conditions
• Perform physical exams, preventive screenings, and health assessments
• Order and interpret diagnostic tests
• Educate patients on wellness, prevention, and disease management
• Deliver compassionate care tailored to the needs of individuals with vision loss and other underserved populations
• Collaborate with behavioral health and interdisciplinary care teams
• Maintain accurate EHR documentation- EMR-NextGen with AI Ambient Assistant

Qualifications Required

• Current or pending Arizona Nurse Practitioner license
• Family Nurse Practitioner certification
• Three years of primary care experience
• Bilingual English/Spanish
