About the Role
As a Social Worker at Avir at Clarksville, you'll work closely with our interdisciplinary team to provide comprehensive care to our patients. Your primary responsibilities will include:
Assessing patients' social and emotional needs and developing personalized plans to address them
Providing counseling, education, and support to patients and their families to promote coping and resilience
Facilitating communication between patients, families, and healthcare providers to ensure continuity of care
Identifying and addressing social determinants of health that may impact patients' well-being
Collaborating with other departments to ensure that patients receive coordinated and comprehensive care
Requirements
Licensed Bachelor or Licensed Master degree in Social Work
